Need help

I am age 55 and had a heart attack. I started eating the right foods, take Simvastatin and a couple of blood pressure meds. I walk at least three times a week for 30 minutes. I have cut my sodium down to about 1800 mg a day and blood pressure is doing great but my triglycerides are too high, HDL is too low, and due to eating properly I am now taking in too much protein so the doctor put me on Allopurinol to prevent gout. I do not smoke or drink. My main problem is the doctor wants me to go on Niacin and I absolutely refuse. What else can I do?
